He said with the help of death audit the department would be able to learn from its past mistakes and not repeat them.</p><p align="justify">He said instructions were given to form a committee at community health centres and primary health centres level. The health centre head would lead the committee and doctors of the health centre would be its members, Dr Thassu added. He further said a meeting would be organised every month to analyse the case history of the patients who died in the month while undergoing treatment in the health centre. A written record of entire discussion would be prepared. The meeting would be organised after the working hours of the health centres. All the details of the meeting would be given to directorate of health department.</p><p align="justify">Similarly, civil surgeons and CMHOs have been instructed to conduct clinical audit in the government hospitals. During this audit treatment given to critically ill patients would be analysed. The analysis motive will be to identify whether the patient is being treated according to the guidelines or not.
