ICMR to fund study on morning-after pill use, abuse by teens & sex workers by Kounteya Sinha

The nation wants to know how often teenagers pop the emergency contraceptive pill.

The Indian Council of Medical Research (ICMR) will support and finance research projects that will look at the pill's use and overuse.

Emergency Contraceptive Pills (ECPs) are used to prevent pregnancy following an unprotected act of sexual intercourse within 72 hours. The council has called for research proposals by December 31 on emergency contraception, especially those that will look at the pill's use and abuse by teenagers and sex workers.

The ICMR wants to document the physical effects of the pill's repeat use, its side-effects and impact on regular family planning methods.

It seeks to know the perception of ECP among women of reproductive age group and its effect on maternal mortality and morbidity. "We want to see the change in prevalence of abortion and illegal abortion after introduction of ECP in the national programme," says an ICMR official.

ICMR says, "The proposals will be peer reviewed and will have to be presented by the principle investigator during the meeting of the council's expert group. Approved research proposals would be supported by the council for funding."

Union health ministry's data suggests that the number of induced abortions is on the wane. However, in absolute numbers, India still records a large number of such abortions.

For instance, in 2008, India saw 6.41 lakh abortions across 12,510 institutions that are approved to carry out medical termination of pregnancy (MTP).

The Family Welfare Statistics in India, 2009, shows 7.25 lakh MTPs were recorded in 2005, 7.21 lakh in 2006 and 6.82 lakh induced abortions in 2007. Going by the 2008 figures, UP recorded the highest number of MTPs at 89,194, followed by Tamil Nadu (63,875), Orissa (59,945), Assam (58,409) and Maharashtra (54,545). While, the corresponding figures in other states like West Bengal are (46,753), Haryana (31,126), Delhi (30,846), Rajasthan (29,292), Gujarat (27,837) and Bihar (24,149). Daman and Diu (42), Andaman and Nicobar Islands (94) and Goa (930) had the lowest abortion rates.

A recent report had said that only two in five of the estimated 6.4 million abortions that occur annually in India are safe. Globally, five million women are treated annually for complications resulting from unsafe abortion.

The exact mode of action of ECPs is not known, but probable mechanisms include prevention of ovulation, fertilization and, or, implantation, depending on the phase of menstrual cycle when it is used.

The mechanisms for prevention of pregnancy are thought to happen before implantation takes place. When used correctly - time and dose as prescribed - after a single act of unprotected sexual intercourse, the ECPs fail to prevent pregnancy in about 2% of women.
