Rural docs toss ‘deliveries’ to central health scheme by Debarati Basu

It is not uncommon for private gynaecologists registered under state government-run Chiranjeevi Scheme to ensure institutional deliveries and refer beneficiaries to government hospitals. But the latest trend is of Chiranjeevi doctors referring cesarean cases to doctors registered under the Central government-run Rashtriya Swasthya Bima Yojna (RSBY) in order to save costs.

Even as doctors registered under Chiranjeevi Scheme have repeatedly complained of the low remuneration package offered by the state government for their services, not much has been done by the state Health Department .

The Chiranjeevi scheme was envisaged to make doctors available in rural areas where proper medical facility is not available, but private gynaecologists were forced to refer the complicated deliveries back to government hospitals.

But the newly launched Central government scheme of RSBY has come as a better option for the Chiranjeevi doctors .

“Being a part of the gynaecology society, we got to know that many Chiranjeevi doctors opt to refer patients with cesarean deliveries to RSBY doctors as the latter are paid more to perform a cesarean delivery over a doctor empanelled under the Chiranjeevi scheme. The low package, delayed payment and various issues have already discouraged many doctors from the Chiranjeevi scheme,” said R B Patel, a member of the Godhra Obstetrics and Gynaecological Society.

A Chiranjeevi doctor is paid Rs 1,545 per delivery irrespective of the type of delivery, while the RSBY offers a doctor a package of Rs 2,500 for normal deliveries and Rs 4.500 for each cesarean delivery.

A private gynaecologist empanelled with Chiranjeevi scheme said on condition of anonymity: “Even if the patient is referred, ultimately she receives an institutional delivery free of cost. Over the last five years, the cost of medical services have increased but the package under the Chiranjeevi Scheme has remained the same. A cesarean delivery does not fit into the budget of a private gynaecologists.”

Officials at the Dahod District Health Department confirmed the reports of patients being referred to doctors attached with the RSBY scheme. The case is the same at Bharuch district too.

“Although not rampant, we have noticed similar tendency in the district. We have been asking the Chiranjeevi doctors to treat the patient who have approached them rather than refer them to RSBY doctors,” said VS Tripathi, Chief District Health Officer, Bharuch

Though all districts are in the process of implementing RSBY, instances of patients being referred have been reported more from districts which were earlier chosen for implementing RSBY in a pilot project.

“Most of the districts have either just recently implemented RSBY or are still in the process of implementing it. So, we have not yet received any such complaints. But RSBY does have a separate section for maternal health and we are trying to register more Chiranjeevi doctors into the RSBY scheme which offers more benefits. Also, it is the choice of the patient to be treated under Chiranjeevi Scheme or RSBY,” said, Harish Goswami, project Coordinator, RSBY, state Health Department.
