                            <div align="justify">-The Hindustan Times</div><p align="justify"><br /><em>Bhopal: </em>Now death audit of every patient who dies in a government hospital would be carried out. The health department's move will help to ascertain the cause of death of the patient. Another reason behind conducting the death audit is to monitor whether there was any negligence in treating the patient.</p><p align="justify">Director, health, Dr KK Thassu said all chief medical and health officers (CMHOs) were instructed to conduct death audit in government hospitals of their respective districts. He said with the help of death audit the department would be able to learn from its past mistakes and not repeat them.</p><p align="justify">He said instructions were given to form a committee at community health centres and primary health centres level. The health centre head would lead the committee and doctors of the health centre would be its members, Dr Thassu added. He further said a meeting would be organised every month to analyse the case history of the patients who died in the month while undergoing treatment in the health centre. A written record of entire discussion would be prepared. The meeting would be organised after the working hours of the health centres. All the details of the meeting would be given to directorate of health department.</p><p align="justify">Similarly, civil surgeons and CMHOs have been instructed to conduct clinical audit in the government hospitals. During this audit treatment given to critically ill patients would be analysed. The analysis motive will be to identify whether the patient is being treated according to the guidelines or not. With the help of this it will be determined if the patient received proper care at every stage of treatment.</p>

                            <div align="justify">-The Hindustan Times</div><p align="justify"><br /><em>Bhopal: </em>Now death audit of every patient who dies in a government hospital would be carried out. The health department's move will help to ascertain the cause of death of the patient. Another reason behind conducting the death audit is to monitor whether there was any negligence in treating the patient.</p><p align="justify">Director, health, Dr KK Thassu said all chief medical and health officers (CMHOs) were instructed to conduct death audit in government hospitals of their respective districts. He said with the help of death audit the department would be able to learn from its past mistakes and not repeat them.</p><p align="justify">He said instructions were given to form a committee at community health centres and primary health centres level. The health centre head would lead the committee and doctors of the health centre would be its members, Dr Thassu added. He further said a meeting would be organised every month to analyse the case history of the patients who died in the month while undergoing treatment in the health centre. A written record of entire discussion would be prepared. The meeting would be organised after the working hours of the health centres. All the details of the meeting would be given to directorate of health department.</p><p align="justify">Similarly, civil surgeons and CMHOs have been instructed to conduct clinical audit in the government hospitals. During this audit treatment given to critically ill patients would be analysed. The analysis motive will be to identify whether the patient is being treated according to the guidelines or not. With the help of this it will be determined if the patient received proper care at every stage of treatment.</p>

                            <div align="justify">-The Hindustan Times</div><p align="justify"><br /><em>Bhopal: </em>Now death audit of every patient who dies in a government hospital would be carried out. The health department's move will help to ascertain the cause of death of the patient. Another reason behind conducting the death audit is to monitor whether there was any negligence in treating the patient.</p><p align="justify">Director, health, Dr KK Thassu said all chief medical and health officers (CMHOs) were instructed to conduct death audit in government hospitals of their respective districts. He said with the help of death audit the department would be able to learn from its past mistakes and not repeat them.</p><p align="justify">He said instructions were given to form a committee at community health centres and primary health centres level. The health centre head would lead the committee and doctors of the health centre would be its members, Dr Thassu added. He further said a meeting would be organised every month to analyse the case history of the patients who died in the month while undergoing treatment in the health centre. A written record of entire discussion would be prepared. The meeting would be organised after the working hours of the health centres. All the details of the meeting would be given to directorate of health department.</p><p align="justify">Similarly, civil surgeons and CMHOs have been instructed to conduct clinical audit in the government hospitals. During this audit treatment given to critically ill patients would be analysed. The analysis motive will be to identify whether the patient is being treated according to the guidelines or not. With the help of this it will be determined if the patient received proper care at every stage of treatment.</p>

Death audit must in govt hospitals now

-The Hindustan Times Bhopal: Now death audit of every patient who dies in a government hospital would be carried out. The health department's move will help to ascertain the cause of death of the patient. Another reason behind conducting the death audit is to monitor whether there was any negligence in treating the patient. Director, health, Dr KK Thassu said all chief medical and health officers (CMHOs) were instructed to conduct death audit in government hospitals of their respective districts. He said with the help of death audit the department would be able to learn from its past mistakes and not repeat them. He said instructions were given to form a committee at community health centres and primary health centres level. The health centre head would lead the committee and doctors of the health centre would be its members, Dr Thassu added. He further said a meeting would be organised every month to analyse the case history of the patients who died in the month while undergoing treatment in the health centre. A written record of entire discussion would be prepared. The meeting would be organised after the working hours of the health centres. All the details of the meeting would be given to directorate of health department. Similarly, civil surgeons and CMHOs have been instructed to conduct clinical audit in the government hospitals. During this audit treatment given to critically ill patients would be analysed. The analysis motive will be to identify whether the patient is being treated according to the guidelines or not. With the help of this it will be determined if the patient received proper care at every stage of treatment.
